Medigap vs. Medicare Advantage: Which Path Is Right for You?
Approaching Medicare eligibility brings an important decision that surprises many people. Original Medicare alone does not cover everything. So, once you enroll, you will likely need to choose between two very different paths for filling the gaps: Medigap or Medicare Advantage.
Both aim to reduce your out of pocket costs, but they work in almost opposite ways. The right choice depends on your health, your budget, and how you prefer to access care.
Medigap: Predictable Costs, Maximum Flexibility
Medigap, also called Medicare Supplement insurance, is designed to work alongside Original Medicare. You keep Parts A and B, and a Medigap policy helps cover the deductibles, copayments, and coinsurance that Original Medicare leaves behind. Medigap plans are standardized in most states, meaning a Plan G from one insurer covers the same core benefits as a Plan G from another, so shopping often comes down to premium and customer service rather than the fine print.
About 42% of people with traditional Medicare carry a Medigap policy, largely because it offers freedom to see any provider nationwide that accepts Medicare, with no networks or referrals required.
The tradeoff is cost. Medigap premiums are paid monthly on top of your Part B premium, and if you apply outside your initial enrollment window, insurers in most states can use medical underwriting to deny coverage or raise your rate.
Medicare Advantage: Bundled Benefits, Lower Premiums
Medicare Advantage, sometimes called Part C, replaces Original Medicare with a private plan that bundles hospital, medical, and often prescription drug coverage into one card.
Many plans also include extras like dental, vision, and hearing benefits that Original Medicare does not cover at all. Around 55% of eligible Medicare beneficiaries are enrolled in Medicare Advantage in 2026, making it the more common choice nationwide.
Premiums are often low or even zero, but plans typically use networks. That means you may need to stay within a specific group of doctors and hospitals or get prior authorization for certain services. Out of pocket costs can also vary more from year to year than with Medigap.
Questions Worth Asking Before You Decide
Before choosing a path, think about how often you travel and whether you want nationwide provider access, how comfortable you are with network restrictions and referrals, whether your current doctors participate in a specific Medicare Advantage plan, and how much budget flexibility you have for monthly premiums versus potential out of pocket costs during a major health event.
Finding Your Best Fit
