A $0 premium Medicare Advantage plan means you will not pay a separate monthly premium to the private insurance company offering the plan.
However, you still pay your Medicare Part B premium, which in 2026 is $202.90 per month for most enrollees, according to the Centers for Medicare & Medicaid Services. The "$0" refers only to the plan's own additional charge on top of that. It is a meaningful distinction that many people miss when comparing options.
The $0 Premium Trade-Off
Here is where the conversation gets important.
$0 premium plans are real and can work well for people who are generally healthy and comfortable with network restrictions. Most of these plans use HMO or PPO structures, which means your choice of doctors and specialists may be more limited than with Original Medicare. If your preferred physician is not in-network, you may pay significantly more or need to switch providers entirely.
